The short answer

Status shows inside your login on the ECHS online portal — and the official FAQ names the office that actually moves your file: your Record Office. Corrections also open only from there. There is no yearly renewal of the card; what exist are one-time replacements (16 Kb compulsory, 32 Kb optional), a five-yearly photo update for minors, and a yearly self-declaration for dependent children above 18. Lost card — two different paths

  1. Lost the new 64-KB card: first block it by SMS from the same mobile number the card was activated from. Then log in, choose Loss of Card, select the members who need a duplicate, and pay only for those cards. Delivery is your choice: paid courier to your home (charges yours) or free collection from Station HQ after the SMS comes.
  2. Lost a 16-KB/32-KB card or temporary slip: a fresh application is filled under the loss category (same form as your chapter), with attachments for each member — but no copy of the lost card is required. A self-declaration of loss is accepted online. The new card is collected from Station HQ; the interim temporary slip issued for the lost card is deposited there.

How do I check my ECHS card application status?

Log in to the ECHS online smart card portal with your registered mobile number and password — the status shows inside your login. The official FAQ adds the part most people miss: the responsibility to verify the application lies with your Record Office, so if it sits at "Awaiting verification by Record Office" for weeks, the FAQ's own advice is to approach your Record Office.

There is a mistake in my ECHS application — how do I correct it?

You cannot edit a submitted application yourself. Per the official FAQ, the edit option is given by your Record Office — wait for it to examine the application; if it raises an observation, you get an SMS, the observation appears in your login, and you can then correct and resubmit.

Does the ECHS card need yearly renewal?

The instructions prescribe no yearly renewal of the 64-KB card itself. What they do prescribe: 16-KB cards stopped working from 1 January 2019 and must be replaced; 32-KB holders may optionally move to the new card for up to 10 years from their card's issue date; photographs of minor children must be updated every five years; and dependent children above 18 submit a yearly self-declaration to the parent polyclinic (white-card holders excepted).

I lost my ECHS card — what do I do?

For the new 64-KB card: block it by SMS from the mobile number it was activated from, then log in to the portal, choose Loss of Card, select the members needing a duplicate, and pay for those cards — with an optional paid courier delivery to your home, or free collection from Station HQ. For a lost 16-KB/32-KB card or temporary slip, a fresh application is filled under the loss category; the copy of the lost card is not required.

How do I change the mobile number registered with ECHS?

For the primary beneficiary: visit your parent polyclinic with the new active number, take a token from the kiosk, and the OIC polyclinic changes it after verifying the smart card. For a dependent: log in on the ECHS website, choose the change-dependent-mobile-number option, enter the new number, and confirm with the OTP that comes to the primary beneficiary's mobile.
